Can triphasic hepatic multislice CT validate the LI-RADS treatment response algorithm after trans-catheter arterial chemoembolization treatment of hepatocellular carcinoma?

Abstract Background Our aim is to evaluate LI-RADS-TR algorithm and its ability assess the viability of TACE-treated HCC. We prospectively evaluated 100 patients with known HCC, treated TACE came for follow-up therapy response plan next step in treatment using triphasic CT study. Imaging was according compared modified Response Evaluation Criteria Solid Tumors (mRECIST) arterial phase hyperenhancement (APHE) criterion. Reference standard “viable” tumors observations included presence strong tumor washout delayed which also shows dense accumulation iodized oil target lesion. Results When equivocal were considered as LR-TR viable, viable resulted 92.31% sensitivity, 83.33% specificity 88% accuracy. On other side when nonviable, it 84.62% while increased 100% accuracy (92%). The mRECIST criteria (presence APHE) showed sensitivity 75%. sensitivities same lesions nonviable lower than specificities higher being 75% mRECIST-viable.
